Please use this form when the troop's activity spans more than six hours and is more than one hour of travel time from the troop's regular meeting location.
Annual Permission Form
Please use this permission slip at the start of a girl joining your troop. It covers much more than just attending meetings at the regular troop meeting times and locations. Please see the form for details.
Health History Form
This form is to be completed for every troop member (girls and adults) and kept with the troop leaders whenever the troop gathers for meetings, field trips, camping, cookie booths, etc.

The service unit is all girls, volunteers, parents and community members that live in a geographic area who work collaboratively to support a strong Girl Scout community.
The Service Unit has several different functions and roles as an extension of Girl Scouts Western Oklahoma such as recruiting new members, supporting and engaging volunteers and delivering programs for girls. Service Units are led by experienced, dedicated volunteers who are appointed and elected to serve in specific positions.
